The Nuckolls County HIS (Human Interagency Services) Team is again sponsoring a “Giving Tree” for residents of Nuckolls County this Holiday season. Please complete this form for you and your family members if you need help with holiday gifts/needs. (NOTE: If you have already completed a form through some other program you can disregard this one.)

Launch the media gallery 3 playerIf you wish to purchase a need for an individual in this county, Giving Tree Tags will be available at the following locations before Thanksgiving:
- Superior Pharmacy & Dollar General in Superior,
- United States Post Office in Nelson, and
- Farmers and Merchants Bank in Lawrence.
Financial donations are always appreciated to purchase unfilled tags and can be sent to: The Giving Tree, PO Box 271, Superior, NE 68978.
If you know of someone in need of items or a food basket, please pass this form along to them. We need your help in meeting the needs of our entire community and county the best we can.
Returned forms will also be used as a basis for distributing Toys-for-Tots, the coat program, and Christmas dinner boxes so please fill out and return if you wish to be included in those programs.
Forms must be returned to the address listed on the other side by October 31, 2019. Requests made after this date may not be filled.
