NSAA Physical Forms Should be Used for ALL SPORTS PHYSICALS

See the attached flyer for more information. Please ensure that you are using the updated NSAA SPORTS PHYSICAL FORM for all sports physicals.
INCOMING PRESCHOOL, KINDERGARTEN, 7TH GRADERS, AND ALL OUT OF STATE STUDENTS WILL ALSO NEED TO HAVE THE DHHS PHYSICAL FORM LINKED HERE, which includes a vision test that is not on the NSAA Physical Form. Also, please note that immunizations will be checked & may need to be updated in these situations as well.
