Flu Shots are Coming to LN Schools on Friday, October 27th
12 PM at Nelson; 2 PM at Lawrence
Launch the media gallery 1 playerPam Bower from Superior Family Medical Center will be at the MS/HS on Friday, October 27th from 12:00-1:30 pm and at the Elementary from 2:00-3:30 pm to distribute flu shots to staff, students and parents, who wish to have the shot.
The cost will be $25 or they can bill your insurance. They will not be able to accept Medicaid, Kid's Connection etc.
See the attachments for further details and consent & payment forms (also put on the front of the school website and handed out at the MS/HS already & will be sent home on Monday from elementary & preschool).
Please print off and fill out the consent and payment form prior to coming to help speed the process up. Those are linked to this post also, just in case you are not able to place the copy brought home by your students.
